Official Registration Checklist

All teams advancing to the 2017 Indiana Science Olympiad State Tournament at Indiana University must have all forms filled out and submitted as outlined in the Official Registration Checklist below in order to compete.

1. Team Pre-Registration Form
   DEADLINE: Teams must submit this form within 7 days of qualifying. 
   This form is to be filled out and submitted online by the coach.
   This form tells us that you are coming and gives us pertinent information that we need to make this tournament a success. Your official team number will be emailed to you within 24 hours of submitting the online pre-registration form.
2. Talent Consent/Publicity Waiver 
   DEADLINE FOR ALL ONLINE TALENT FORMS: March 10
   Every teacher, student, parent, and chaperone must submit an online Talent Consent/Publicity Waiver form. Please note if someone from your team opts not to fill-out this form, our photography & video crews will avoid taking pictures of your entire team while on campus and you will not appear in the "Reflections Video" shown at the awards ceremony. 
3. Medical Consent Form
   To be filled out by a parent or legal guardian.
   Every participating student must have this form filled out.  
   NO team will be allowed to compete without having one form per student in attendance - so be sure you have one form per student attending.
   This form will be collected on tournament day at the Team Registration area.
4. Release from Liability Form
   To be filled out by a parent or legal guardian.
   Every participating student must have this form filled out.  
   NO team will be allowed to compete without having one form per student in attendance - so be sure you have one form per student attending.
   This form will be collected on tournament day at the Team Registration area.
5. Code of Ethics
   Print and have the entire team including coaches, competitors, alternates, observers, parents sign the Code of Ethics form.
   This form will be collected on tournament day at the Team Registration area.
6. Student Registration Form
   NEW THIS YEAR!
   Every student (competitors and observers) must fill-out this online student registration form.
   Students will need to know if they are a PRIMARY COMPETITOR or an OBSERVER.
   They will also need to know their State Tournament assigned TEAM NUMBER.
7. Final Registration Form
   Coach AND Principal must fill-out and submit the online Final Registration Form.
   You WILL need full name, grade, and birth date for every student.
   You do NOT need a username and password this year to fill out this form.
   However, you MUST have your school principal's electronic signature.
